Please arrive 15 minutes prior to your first scheduled appointment time so we can process your paperwork. Our new patient's forms can be downloaded using the link below.

We are a Delta Dental Premier provider. We also accept most Dental PPO plans as an out-of-network provider. Please provide us with your insurance as soon as possible prior to your appointment.

Please note that payment is required at time of service.
